THE PATH TO

PARTNERSHIP

The step-by-step guide to apply for a service dog.

1: JOIN THE RANKS

Before moving forward with an application, it’s important to do a simple self pre-screen.

Though we strive to help as many Veterans as possible, we need to be sure both you and your potential 4-legged friend are right for each other.

If you are looking for more information prior to applying, you can find more by visiting the ADA website.

SELF PRE-SCREENING

Review the self assessment questions below to determine if you are a candidate. If you answer ‘YES’ to all of these questions, then you qualify to move on!

Were you honorably discharged?

Are you at least 50% disabled?

Are you a Texas resident?

Is your record free of felony assault charges?

Open the fillable PDF and fill it out entirely.

Upon completion, email the application and all supporting documents in a single submission to Sarah Kocurek.

2. OUR SELECTION PROCESS

Upon receiving your full application & supporting documents, we’ll begin the process of finding your perfect service dog and new best friend.

We want to get to know you, learn your needs, and determine how a trained service dog can be of assistance.

PHONE INTERVIEW

We’ll schedule a phone interview to discuss your specific needs and outline clear expectations for the remainder of the process.

VIRTUAL HOME VISIT

Next, we will arrange a virtual home visit via FaceTime to discuss your future companions living environment.

BASIC DOG TRAINING

Finally, we’ll focus on finding you a dog that has completed training, is ready for a partner, and matches your personality and needs.

3. THE CHOSEN FEW

When we find you a dog that is ready for a partner, we will give the final approval. This will signal the start to a wonderful friendship!

APPROVED!

Approved applicants will then require 3-4 weekends (Saturday & Sunday) to gain quality handler and Public Access training, become familiar with basic dog training, and build rapport with your new service dog.

GRADUATION DAY

Upon completion of the training, you now have a full-fledge service dog!

On Graduation Day, an independent trainer will test you and your new dog in a public place. A special ceremony will follow the successful evaluation, recognizing your commitment and service. Your family is invited to share in this moment as we present you a diploma and a Quilt of Valor.
